Healthcare providers who use ImPACT®, ImPACT Pediatric®, ImPACT Quick Test® or Cognitive Impairment Screener™ (CIS) can access clinical reports using the ImPACT Passport ID lookup feature in the Customer Center. ImPACT Passport gives patients a secure place to store their ID so it’s available when you need it.

SECURE DATA

Test takers grant access to healthcare providers by supplying their unique ImPACT Passport ID. Patient data can only be accessed by healthcare providers via the Customer Center.

EFFECTIVE REPORTING

The ImPACT Passport app gives patients the option to track their symptoms by answering a series of short questions. Real-time symptoms reports can be accessed in the Customer Center.

OBJECTIVE CARE

Avoid waiting for transfers and get instant access to patient data including clinical reports and symptoms even if testing was administered at another location.

ImPACT PASSPORT FAQs

How do I transfer patient test scores to my account?

Looking up patient data via their ImPACT Passport ID in the Customer Center does not automatically transfer their information to your database. You will need to complete a data transfer form if you want the data to be accessible in your account.

Can I issue a post-injury test under an existing ImPACT Passport ID?

Yes. When launching a follow-up exam from the Customer Center select “Start New Test”, “Previously Tested-search by Passport ID” and enter in the individual’s ImPACT Passport ID. Select the test taker’s name, the test type, and click “Launch Test”. Upon completion, the test will be saved in your database under the original ImPACT Passport ID number.

How can I find a patient’s ImPACT Passport ID?

If a patient cannot provide their ImPACT Passport ID, they should contact the organization that issued their test. If the organization no longer exists, the patient will need to send ImPACT Applications a notarized statement requesting a copy of their medical records.
